When an adolescent or young adult leaves treatment, they are faced with a different set of challenges than an adult.

RCP’s Transition Coaching provides individualized support for academics, life skills augmentation, executive function, resume writing, job searching, interviewing, integration to appropriate 12-Step community, random screening, and managing the challenges of “partying” as the social norm.

 

Transition Coaching Tier I

  • Case Management for recovery, academic, and social progress

  • Weekly 1 hour face-to-face meeting at the beginning of the week for progress update and mentoring

  • Weekly 1 hour face-to-face meeting at the end of the week, followed by accompaniment to one 12-step meeting, followed by up to an hour of fellowship

  • Nightly phone check-in

  • Weekly parent call to review progress and challenges

Transition Coaching Tier II

  • Case Management for recovery, academic, and social progress

  • Weekly 1 hour face-to-face meeting for progress update and mentoring

  • Accompaniment to one 12-step meeting, followed by up to an hour of fellowship

  • Three times per week phone check-in

  • Twice monthly parent call to review progress and challenges

Transition Coaching Tier III

  • Case Management for recovery, academic, and social progress

  • Weekly 1 hour face-to-face meeting for progress update and mentoring

  • Weekly phone check-in

  • Monthly parent call to review progress and challenges

There is no obligation for New Points’ residents to contract these services. The services are not provided by New Points nor are they provided on-site.
