RCP’s Case Management is a simple accountability structure for the continuing care plan established when the client left in-patient treatment. A recent study conducted by the Recovery Research Institute states, “…authors found that brief telephone check-ins reduced substance use and rates of readmission to detoxification treatment in the three months after discharge.”
Case Management Tier I
- Daily phone check-in with client 
- Weekly phone check-in with client and concerned others 
Case Management Tier II
- Weekly phone check-in with client 
- Monthly phone check-in with client and concerned others
